You need a Terms of Use, Privacy Policy and Disclaimer on your website. I have not personally used the templates from these shops, but I have researched them for you and they have products that specifically serve a wide range of niches, including health and wellness (personal training, doula, health coaching etc.). Please use these as starting points but perform your due diligence before choosing a legal bundle!

They offer a Website bundle containing Terms of Use, Privacy Policy and Health Disclaimer. They also offer a Fitness and Wellness bundle which includes client contracts.
